SAP System Outage: Injury Reporting

Notice of Planned SAP Outage from 5 p.m. on Friday, December 2, 2022 until 6 a.m. on Monday, December 5, 2022. 

If an incident occurs during the planned SAP Outage, please submit Supervisor Injury Reports by following the instructions below:

  • For a more serious accident involving injury where 1 or more people are admitted to the hospital as an inpatient, where the injury involves an amputation or a loss of an eye, or one which results in death, please contact the MIT Operations Center by calling (617) 253-1500, or (3-1500) and they will contact the EHS Office. Supervisors will then need to submit the Supervisor’s Injury Report (see link below) within 8 hours of the accident. If you fill out the Supervisor’s Injury Report via the Word doc form, save the document to your desktop and email it to the OSHA Record Keeper, sma@mit.edu.
  • For a less serious incident that does not meet the criteria above, please follow the link provided to open the Supervisor’s Report of injury electronically, save the word document to your desktop and email it to the OSHA Record Keeper, sma@mit.edu within 24 hours.
  • If you print out the report and fill it out manually, please fax it to the EHS Office at 617-258-6831.
